RESUMO

In hepatolenticular degeneration, also known as Wilson's disease (WD), the binding of copper to ceruloplasmin is impaired, resulting in the deposition of free copper mainly in the liver, brain, and other organs. The prevalence rate of WD was 1/30 000. In clinical practice, most WD patients are young adults and are characterized by extrapyramidal symptoms, liver cirrhosis, Kayser-Fleischer ring of the cornea, low serum ceruloplasmin level ( 100 mg/L). At present, pharmacotherapy is the main treatment method for WD, and in some cases of end-stage liver disease or acute liver failure, liver transplantation is an option to improve survival rate. This article reviews the research advances in the pathogenesis of hepatolenticular degeneration, phenotype-genotype relationship, and pharmacotherapy in China and globally.

RESUMO

Objective To investigate application value and significance of mixed reality technology in surgical treatment and doctor-patient communication for vestibular schwannoma. Methods We selected randomly 13 vestibular schwannoma patients treated with surgical treatment. After the three-dimensional models were constructed, preoperative surgical planning and doctor-patient communication were performed with mixed reality technology. Craniotomy through retrosigmoid sinus approach, tumor resection and facial nerve protection were achieved intraoperatively with the assistance of mixed reality technology. Questionnaires were collected and facial nerve function of 13 patients was recorded one week after operation. Results Holographic model images of 13 cases were showed successfully using mixed reality technology. The locations of preoperative facial nerves reconstructed were completely consistent with actual locations in 10 cases (84.6%). After preoperative anatomic analysis, it was decided to remove partly the posterior wall of the internal auditory canal in 11 cases. The result of doctor-patient communication questionnaire showed that 13 patients and their family all had a thorough understanding of the condition, operative plan and risks, and expressed satisfaction with the preoperative conversation. With the assistance of mixed reality technology, the tumors were resected totally without injury of vein sinus in 13 cases. The facial nerve function was gradeⅠin 3 cases, gradeⅡin 6 cases, grade Ⅲ in 3 case and grade Ⅳ in 1 case based on House-Brackmann grading one week after surgery. Conclusion Mixed reality technology is quite helpful in individual surgical planning and preoperative doctor-patient communication. It helps reduce the side injuries of surgery and protect the function of facial nerve as a surgical assistant tool intraoperatively.

RESUMO

@#Hyaluronic acid, also called hyaluronan(HA)is a biocompatible and biodegradable linear polysaccharide which is of interest for tumor targeting through cell surface CD44 receptors. It is widely applied in the field of tumor therapy as an anticancer drug delivery carrier, and has become a hot spot in the research of tumor targeted drug delivery system. In tumor drug therapy, the key to reduce toxicity is to actively target tumors by using anatomical, pathophysiological and microenvironmental differences between malignant tumors and normal tissues. Differentiation cluster 44(CD44)is a high-affinity receptor for HA, which can be marked as a tumor marker or a targeting receptor because it is overexpressed in tumor cells. The overexpression of CD44 receptors was observed in many tumors such as breast cancer, colorectal cancer, liver cancer and pancreatic cancer. The effect of hyaluronic acid drug nanocarriers on various tumors is briefly described, indicating that the overexpression of CD44 receptor is an ideal choice for the treatment of hyaluronic acid-based drug carriers. The CD44 ligand can increase the affinity of the nanocarrier for tumor cells by binding to the nano drug carrier. The HA structure is known for its potent tumor targeting effect due to the inclusion of CD44 ligand, which enhances uptake of tumor cells by the HA-CD44 receptor-mediated endocytosis pathway. The study reviewed the progression of hyaluronic acid nanomicelles in clinical tumor therapy and its release behavior. The percentage of drug release and release rate are the key factors in the overall efficacy of the treatment strategy. Therefore, a great number of studies have focused on inducing drug release in Cytosol by taking advantage of the difference between the internal and external environments of nanostructured micelles or through external stimulation post-treatment applications. The study proved that an acid environment is more favorable to achieve a greater release and drugs can be quickly and completely released in an oxygen-deficient environment. In addition, the great potential of hyaluronic acid nanomicelles in tumor therapy was also further identified in this article. In vitro and in vivo experimental studies have repeatedly shown that hyaluronic acid-based nanomicelles are a drug- and gene-specific targeting tumor delivery method, in combination with passive targeting, this active targeting strategy is a promising approach to providing chemotherapy drugs to CD44 overexpressing tumors. In conclusion, hyaluronic acid-based nanomicelles are biologically safe with great potential to drug release, blood compatibility and systemic tumor targeting which all implied it has good application prospects in clinical tumor treatment.

RESUMO

OBJECTIVE:To investigate the protective effect and mechanism of Wei medicine Cichorium glandulosum 95% eth-anol extract(CG-I)on immunological liver injury in mice, and provide reference for post-screening its effective site. METHODS:60 mice were randomly divided into blank control group(normal saline),model group(normal saline),positive control group(Di-ammonium glycyrrhizinate,100 mg/kg) and CG-I high-dose,medium-dose,low-dose groups (calculated by crude drugs as 200, 100,50 g/kg),10 in each group, intragastrically administrated once every day,for 10 d. After 1 h of last administration, except for blank control group, mice in other groups were intravenously injected Con-A in tail to induce immunological liver injury. After 8 h of modeling, tumor necrosis factor α(TNF-α),interferon γ(IFN-γ),interleukin 1β(IL-1β)contents in serum were detected;liver and spleen indexes were calculated. The pathological changes in liver tissue were observed, and aspartate aminotransferase(AST), alanine aminotransferase (ALT), glutathione S-transferase (GST), alkaline phosphatase (AKP), total superoxide dismutase (T-SOD),malondialdehyde(MDA)levels in liver tissue were detected. RESULTS:Compared with blank control group,TNF-α, IFN-γ,IL-1β contents in serum in model group were significantly increased;liver, spleen indexes and AST,ALT,GST,AKP, T-SOD levels in liver tissue were significantly increased;and MDA level in liver tissue was significantly reduced, with statistical significances(P<0.05 or P<0.01);liver of mice in model group was cluttered, showing swelling, necrosis and other diseases in liver cells. Compared with model group, except that AST, ALT,AKP levels in liver tissue in CG-I low-dose group and MDA, IFN-γ contents in serum in CG-I medium-dose, low-dose groups had no significant decrease, other indexes were significantly improved (P<0.05 or P<0.01);and patho-logical changes in liver tissue were relieved to varying degrees. CONCLUSIONS:CG-I shows protective effect on Con-A-in-duced immunological liver injury in mice, especially the high dose and medium dose. The mechanism may be associated with its an-ti-oxidation and anti-inflammatory effects.

RESUMO

<p><b>OBJECTIVE</b>To study the effects of RhoA down-regulation by RNA interference on the invasion of tongue carcinoma Tca8113 and SCC-4.</p><p><b>METHODS</b>Determination of the human RhoA sequence as well as the design and constructionof a short specific small interfering RNAs (siRNA) were performed. The siRNA of RhoA gene was transfected into humantongue squamous cell carcinoma Tca8113 and SCC-4 cells line by Lipofectamine 2000. Quantitative real-time polymerasechain reaction was used to examine the mRNA expressionlevels of RhoA. Protein expressions of mRNA, galectin-3,and matrix metalloproteinase (MMP)-9 were evaluated byWestern blot. Transwell invasion assay was performed toassess the invasion ability of tongue carcinoma.</p><p><b>RESULTS</b>RhoA expressions in Tca8113 and SCC-4 cells were reducedsignificantly after transfection of RhoA-siRNA. Protein levels f galectin-3 and MVP-9 were also down-regulated significantly. Invasion ability was inhibited as well.</p><p><b>CONCLUSION</b>RhoA-siRNA can effectively inhibit RhoA expression in Tca8113 and SCC-4 cells. The invasion ability of tongue carcinoma cells decreased with down-regulation of the protein expressions of galectin-3 and MMP-9, indicating that RhoA-siRNA can inhibit invasion of tongue carcinoma. Results show that RhoA may play an important role in the processes of invasion and metastasis of tongue carcinoma.</p>

RESUMO

Forty-eight male Wistar rats, weighing 280 to 320 g,were selected and prepared by fasting but free drinking for 12 hours before the experiment. They were randomly divided into four groups by random number table (12 rats in each group):control group, ischemic group, reperfusion group and propofol group.Control group:normal saline infusion[10 ml /(kg?h)]for 1 h in caudal vein without any other treatment.Ischemic group:on the basis of control group, supply with 20 mmHg(1 mmHg =0.133 kPa)pneumoperitoneum pressure for 1 h. Reperfusion group:on the basis of ischemic group, keep pneumoperitoneum pressure open for 30 min.Propo-fol group:infusion propofol in caudal vein [10 ml /(kg?h),saline 10-fold dilution],given 20 mmHg pneu-moperitoneum pressure for 1 h,then kept this pressure open for 30 min.The expression changes of blood urea nitrogen(BUN),creatinine(Cr),superoxide dismutase(SOD),malondialdehyde(MDA)and nuclear factor E2-related factor 2(Nrf2)were detected individually. Results The levels of BUN,Cr and MDA gradually increased, while the level of SOD decreased in control group, ischemic group and reperfusion group (P ﹤0.05,respectively).Compared with ischemic group and reperfusion group, the levels of BUN,Cr and MDA decreased significantly, and the level of SOD increased markedly in propofol group(P ﹤0.05,respectively). The expression of Nrf2 protein and mRNA gradually increased in the control group, ischemia group,reperfu-sion group and propofol group (P ﹤0.05).Conclusion Carbon dioxide pneumoperitoneum can cause renal IRI in rats.Propofol has a protective effect of IRI by regulating the expression of Nrf2 protein.
